Cancelled: Smart Health Communities and the Future of Health in Philadelphia

Smart health communities is a futuristic view of healthcare leveraging digital technologies to create smart healthcare communities in support health and wellness. The focus of this program is creating smart health communities that also factor in addressing the needs of Social Determinants of Health (i.e. housing and food insecurity, transportation, and other factors that impact the overall health care outcomes for patients).

Smart health communities (SHCs), are defined as containing some or all the following core features:
1. They empower individuals to proactively manage their health and well-being;
2. They foster a sense of community and belonging;
3. They use digital technology and behavioral science;
4. They use data to meaningfully improve outcomes; and
5. They create new and innovative ecosystems.
