 I agree to the event cancellation policy.*
Cancellation policy
Confirmed registrants may cancel and receive a full refund (minus any administrative fee) up to 10 days prior to the event date. A completed cancellation form  must be received in writing via email to hba@hbanet.org no later than 10 days before the event date. Registrants who do not cancel and do not attend will be responsible for the full registration fee.

Confirmed registrants may cancel and receive a full refund (minus a $75 administrative fee) until 29 October 2021. A completed cancellation form must be received in writing via email to hba@hbanet.org no later than 29 October 2021. Registrants who do not cancel by 29 October 2021 and do not attend will be responsible for the full registration fee.

If a portion of your registration fee is covering HBA membership dues ($175 USD) this is nonrefundable and nontransferable per the HBA policy.

 I agree to the event substitution policy.*
Substitution policy
Confirmed registrations may substitute their registration up to 10 days prior to the event date. A completed substitution form  must be received in writing via email to hba@hbanet.org no later than 10 days prior to the event date. After such time, all registration substitutions must be made on-site at the event.

Confirmed registrations may substitute their registration by 29 October 2021. A completed substitution form must be received in writing via email to hba@hbanet.org no later than 29 October 2021. Substitutions made after 29 October 2021 through 10 November 2020 are subject to a $75 administrative fee. Substitution to a nonmember will require paid membership to the HBA in addition to the substitution fee.

Fitness waiver
The Healthcare Businesswomens Association (HBA) requires that all persons who participate in fitness events do so at their own risk and that each participant agrees to the following agreement prior to taking part in the activity and urges all participants to act in such a manner as to protect their own safety and the safety of others.

I fully understand that my participation in the program is purely voluntary. I desire to participate and agree to act in a reasonable and prudent manner under the circumstances. I further understand that any HBA volunteers participating in the activity will be acting in an individual capacity only and will not represent the HBA. As a condition to my participation in the activity, I waive and release the HBA and their officers, employees and agents from any claim that I may have for any damage to property or injury to my person that may result in whole or in part from my participation in the activity. Also, I agree to allow the HBA to use any photographs of me for marketing and advertisement purposes.
Attendee media
Registrations and attendance at, or participation in the following event constitutes an agreement by the registrant to the use and distribution (both now and in the future) by HBA of the registrant or attendee's image or voice in photographs, videotapes, electronic reproductions and audiotapes.
Attendee privacy and communication
Registration and attendance at, or participation in the following event constitutes an agreement by the registrant to the use of data by HBA related to this event, including purposes such as event specific communications and marketing, necessary logistical planning procedures including personal demographics and preferences.
Privacy Policy

Registration and attendance at, or participation in, HBA meetings and other activities constitutes an agreement by the registrant to the use and distribution (both now and in the future) of the registrant or attendee’s image or voice in recordings, both live and on-demand, photographs, videotapes, electronic reproductions and audiotapes of such events and activities by HBA and other third parties including but not limited to the venue, the host city, and the host CVB or virtual platform provider. Your virtual registration may include technology that monitors your activities throughout the meetings, such as session attendance and virtual booths visited.
